How dental care works in the Netherlands, and where aligners fit in

Nobody hands you the manual when you move here. People who grew up in the Netherlands picked all of this up without noticing, so it rarely gets explained out loud. You're simply expected to know it.
So, the short version. You arrange your own dental care, and you register with a practice yourself. From the age of 18 you generally pay for dental and orthodontic care out of your own pocket rather than through your basic health insurance. Straightening teeth is a separate field. Own plan, own provider, own price.
Who does what in Dutch dental care?
Three Dutch words cover most of it.
- Tandarts. Your general dentist. Check-ups, fillings, pain, the everyday work.
- Mondhygiënist. The dental hygienist, who looks after your gums and the cleaning.
- Orthodontie. The field that moves teeth. Planned and followed by a provider who does this work every day.
That last one is the part people miss. Moving teeth isn't something your regular dentist adds on at the end of a routine check-up. It's a treatment of its own, with its own appointments and its own plan from start to finish.
Do you need a referral to get started?
No, and that surprises a lot of people who come from systems built around a family doctor.
Nobody assigns you a dentist here. You register with a practice yourself. Your huisarts, the Dutch family doctor, is the gatekeeper for a lot of medical care. Dental care sits outside that route.
For aligners you can book a first appointment directly. You never need a referral. You also don't need to be an existing patient with us. And if you already have a tandarts you like, keep them. They look after your teeth in general, and a separate provider looks after moving them.
Does your Dutch health insurance pay for it?
Probably not in full, and often not at all. This is where expectations from another country cause the most confusion, so it's worth being precise.
Your Dutch basic health insurance, the basisverzekering, is not a dental plan. It's built around doctors and hospitals. For adults, dental and orthodontic care sit outside it, apart from a few specific medical situations.
What can help is additional dental cover, an aanvullende tandartsverzekering, which many people take out on top of the basic package. It often pays part of the cost, up to a maximum per year.
Two practical things to do:
- Search your own policy for the Dutch word orthodontie. That's where your percentage and your annual maximum are written down.
- If the wording is unclear, ask your insurer to confirm in writing what they cover for an adult before you start.
Policies differ per insurer and change from year to year. So treat your own policy as the source of truth, rather than anything you read online, including this page.
Looking for an orthodontist in the Netherlands who works in English?
Type that into a search bar and you get back dental practices, clinics, aligner brands, all using slightly different words for what they do. Working out what you're actually comparing is the hard part.
Two things are worth checking before anything else. First, whether someone looks at your teeth in person and stays involved while they move, rather than sending you a box and checking in remotely. Second, how much experience sits behind the plan. Comfortho has been active since 2008, has been an Invisalign Elite Provider since 2016 and sits in the global top 1% of providers, with more than 2,885 treatments completed.
Everything on this site is available in English, and you can get in touch in English.
Where does an aligner treatment fit in?
An aligner treatment isn't a product you order. It runs like this.
- Your teeth and gums are looked at first, because healthy gums come before movement.
- A digital scan is made. No trays of impression paste, just a scanner.
- You get a plan, and you hear what it would mean in your case before you decide anything.
- You wear a series of clear aligners, each one a small step, and someone follows how your teeth respond.
- After the last aligner, a retention phase keeps everything in place while your teeth settle. That runs for a longer period, not a few weeks.
Most treatments run 8 to 16 months. How long yours takes depends on how much has to move, and you'll hear that after the scan.
The full explanation of how the treatment itself works is on our Invisalign page.
What does it cost?
At Comfortho a full treatment costs between 3,395 and 4,885 euros. Where you land in that range depends on how far your teeth need to move and how long your treatment takes. A lighter correction sits at the lower end. A full treatment of both jaws sits higher.
You hear your own figure after your teeth have been looked at, never as a guess over the phone. The details are on the Invisalign costs page.
One piece of advice that applies anywhere you go, including here. Ask every provider what is and isn't included in the price they quote. Aligners, the check-ups along the way and the retainer at the end are not always covered by the same number.
